GRADEシステムにおいては、エビデンスの質評価基準の中のLimitations（限界）が深刻なものであればあるほど、エビデンスの質の低下になる。
具体的なlimitation判定項目としては、

■ランダム化比較試験[RCT]での Limitations

• lack of allocation concealment
• lack of blinding (particularly if outcomes are subjective and their assessment highly susceptible to bias)
• Incomplete accounting of patients and outcome events (large loss to follow-up)
• Selective outcome reporting (Reporting of some outcomes and not others on the basis of the results)
• stopping a trial early for benefit
• use of unvalidated patient-reported outcomes

■観察研究でのlimitations

• Failure to develop and apply appropriate eligibility criteria (inclusion of control population)
• Flawed measurement of both exposure and outcome
• Failure to adequately control confounding
• Incomplete or inadequately short follow-up

また、研究のタイプや内容によっては、上記の項目以外のlimitationsになりうるものがある。ガイドラインパネルや系統的レビューの作成者はあらゆるlimitationsの可能性を考慮すべきである。
もし、limitations(限界)が、深刻なものであるならば、エビデンスの質は１段階あるいは2段階、等級ダウンとなるかもしれない。
